Effective Date: January 1, 2025
This UA Program Agreement ("Agreement") is entered into by and between Patriot Pixels LLC, a Florida limited liability company ("Company," "we," "our," or "us"), and the participant ("Client," "you," or "your"). By enrolling in the UA Program, you agree to the terms and conditions outlined below.
The UA Program is a custom website offering designed specifically for licensed insurance agents affiliated with, or operating similarly to, agents of USHEALTH Advisors ("USHA"). This program provides a no-upfront-cost website in exchange for a minimum three (3) month subscription to one of our hosting plans.
* Patriot Pixels LLC is not affiliated with, endorsed by, or officially connected to USHEALTH Advisors or any of its parent or partner companies.
* The use of the term "USHA agents" is solely for identification of the target audience and does not imply any formal affiliation or endorsement.
* All trademarks, logos, and names associated with USHA remain the property of their respective owners. Patriot Pixels makes no claim to them.
* By participating in the UA Program, you acknowledge and accept that there is no official relationship between Patriot Pixels LLC and USHA.
The UA Program includes the following features, subject to the terms of this Agreement:
* Custom Domain Name: Published on a standard domain (e.g., yourname.com) if available.
* Custom 3-Page Website: Typically includes Home, About, and Contact pages.
* Mobile-Responsive and SEO-Optimized Structure: Optimized for all devices and basic on-page SEO.
* Custom Contact Form: Configured to forward submissions to your designated email.
* Stock Image Usage: We may use royalty-free images where client-provided assets are unavailable.
* Limited Design Revisions: Reasonable edits are included during the design phase, subject to our Acceptable Use Policy.
Clients may request additional features beyond the scope of the standard 3-page design. These are available as one-time purchases or monthly installments.

All add-ons are subject to availability and require upfront payment or subscription, based on our official pricing sheet.
To qualify for the UA Program, you must:
* Be a licensed U.S.-based insurance agent affiliated with or similar to agents of USHA
* Submit an application and be approved by Patriot Pixels LLC
* Provide all required content and brand materials in a timely manner
We reserve the right to deny or revoke participation at any time, for any reason.
You must subscribe to one of our hosting plans (starting at \$69/month) prior to project commencement. A minimum 3-month commitment is required.
If you cancel before completing the 3-month term, you agree to pay the difference between your total payments and the full 3-month minimum. This fee must be paid within seven (7) calendar days of cancellation. Failure to pay may result in account suspension and/or legal collection.
* The website, its design, layout, and code remain the property of Patriot Pixels LLC.
* You are granted a limited, revocable license to use the site while subscribed to an active hosting plan in good standing.
* You retain ownership of all logos, media, and content you provide to us.
* Use of our work beyond this Agreement without written permission is prohibited and may result in legal action.
You agree to:
* Provide all content, brand materials, and information in a timely manner
* Respond promptly to design drafts and revision requests
* Not use the website for any unlawful, deceptive, or abusive activity
* Comply with our Acceptable Use Policy and all other legal documents at [https://www.patriotpixels.net/legal](https://www.patriotpixels.net/legal)
Websites are typically completed within 7 business days after your approval and all content is submitted. Delays in providing materials will result in timeline delays. Revisions are permitted within reason and governed by our Acceptable Use Policy.
We reserve the right to modify or terminate the UA Program at any time without prior notice. Continued participation constitutes agreement to the most current version of this Agreement.
To the fullest extent permitted by law, Patriot Pixels LLC shall not be liable for any indirect, incidental, or consequential damages arising out of this Agreement or use of the website.
This Agreement is governed by the laws of the State of Florida. Any disputes shall be resolved in Florida courts.
This Agreement constitutes the entire agreement between the parties regarding the UA Program and supersedes all prior agreements, oral or written.
By participating in the UA Program, you acknowledge that you have read, understood, and agree to be bound by the terms of this Agreement.
